Re-entry services

I was recently released and want to be connected with a re-entry program, who should I call?

There are many re-entry programs in NYC that may be able to provide support and services. The Fortune Society (212) 691-7554) and Osborne Association are the largest reentry providers in NYC. Osborne has locations in the Bronx (718-707-2600), Brooklyn (718-637-6560) and Manhattan (212-324-5577).

In addition, Housing Works, CASES, CEO, and PRI have great programming options.

I was just released from detention and need my prescriptions, what should I do?

You can call the Correctional Health Services (CHS) Point of Reentry and Transition Practices (PORT) at 646-614-1000 or email CHSPORTline@nychhc.org for help with prescriptions. PORTline is also available to troubleshoot problems with Medicaid coverage (e.g. suspended/in patient only status). They can also provide referrals to other social service and health care providers.
